EDENBRIDGE ADULT’S PROGRAM
Supported Independent Living
and need only limited support in a few areas of life.
Individuals living in their own home who will receive Supported Independent
Living services will be supported hourly to live and participate in the community
as independently as possible. These services will be developed strictly on an
individual basis and will reflect following: Community Inclusion, Freedom,
Respect, Responsibility, Location, Informed Choice, Self-Determination, and
Permanency.
Support Homes
living skills may choose a Supporting Family or Supporting Roommate setting.
These supports are provided in a home where the living arrangements are shared
between the support worker and the individual. A Support Home provides
individualized services that encourage individual responsibility, freedom to make
choices, as well as the opportunity to develop independent living skills. Support
homes are chosen with a focus to developing positive community relationships and
are congruent with: Community Inclusion, Freedom, Respect, Responsibility,
Location, Informed Choice, Self-Determination, and Permanency.
Respite
their family or primary support worker requires some time off from their regular
schedule. These supports are an extension of the supports that an individual
receives while in their support home. Individuals continue to receive
individualized supports that provide opportunity for personal growth, to develop
independent living skills, encourage individual responsibility, as well as freedom
to make choices.
Out-of Home Respite is provided for individuals who may require occasional
overnight support while their family or primary support worker requires some time
off from their regular schedule. These supports are an extention of the supports
that an individual receives while in their support home. Individuals continue to
receive individualized supports that provide for the opportunity for personal
growth, to develop independent living skills, encourage individual responsibility,
as well as freedom to make choices.
Overnight Staffed Residence
than can be provided in a Support Home. These arrangements offer up to 24 hour
support in an environment that is considered to be their own home. Individuals
receiving supports in these settings will also be provided opportunity to make their
own choices as well having appropriate responsibilities. This environment
encourages personal progress as well as building important community
relationships. These services will also be developed strictly on an individual basis
and will reflect the following: Community Inclusion, Freedom, Respect,
Responsibility, Location, Informed Choice, Self-Determination, and Permanency.
Edenbridge ensures these supports and choices are taking place through monthly
(or more frequent as needed) meetings with the individual and their support
worker. Documentation of these meetings is kept and guardians are provided with
a monthly summary of services.
Community Access
emphasis on accessing community resources and building relationships as they
participate in various activities. Individuals are supported with exploring their
interests and with pursuing their goals around developing personal skills and
abilities. The Community Access activities may have a recreation or volunteer
component in the community. The focus of these supports is to promote full and
inclusive lives for individuals, to assist in developing valued societal roles, and
reflect the following: Community Inclusion, Freedom, Respect, Responsibility,
Location, Informed Choice, Self-Determination, and Permanency.
